We are excited to announce the expansion of our physical location at 33 Bernhard Road, North Haven effective October 1st, 2026!
This expansion represents more than additional space, it represents growth, opportunity, and our continued commitment to the individuals and families we serve.
As Transforming Lives continues to grow, our expanded space will allow us to enhance our programming, welcome additional individuals, and create new opportunities for learning, skill development, social engagement, and meaningful community participation.
The expanded location will include additional program areas, a kitchen, common space, and other dedicated areas designed to support engaging, person-centered activities and services. Most importantly, the expansion will give our team greater flexibility to develop programming that reflects the interests, goals, abilities, and needs of the individuals we support.

We encourage DDS case managers, families, residential providers, and individuals seeking services to contact us to learn more about our available programming and opportunities for both small- and large-group Day Support Options (DSO/DSH) and other individualized supports.
